My boss and I are wondering if there is way for us to auto-populate the current location where a sales rep is completing a survey?
For example, can we pick up there street address, zip code, country, state, etc... when they open a new survey form? And get a timestamp?

I dont think so you can access the address when you taking survey offline..
However, you can use the Salesforce Offline feature to store the survey and store address.
